- 15% OFF Additional Cleanings, Periodontal Maintenance, Dental Sealants, Fillings, Core Buildups, Oral Surgery
- 15% OFF Crowns, Veneers, Periodontics, Root Canals, Dentures, Partials, Implants

Program Guidelines
- Patient’s portion of bill is due the day of service.
- Cannot be used in conjunction with another dental plan, dental insurance, or financing program such as CareCredit.
- No refunds or premiums will be issued at any time if participant decides not to utilize dental plan.
